Job Description:
This physician job involves providing direct patient care in accordance with evidence-based treatment guidelines. The physician will deliver care using a team-centered approach and interact with patients and families to keep them informed and engaged in healthcare decisions. The role includes counseling patients about wellness, illness prevention, and health maintenance while referring them to specialty care services and educational training programs when required. Physicians will also participate in a Patient Centered Medical Home model to improve the health outcomes of patients. Key Responsibilities:
- Provides direct patient care to patients at the practice site in accordance with evidence-based treatment guidelines in a Patient Centered Medical Home model.
- Delivers well care and uses a team-centered approach to manage chronic disease states to improve the health of patients.
- Interacts with patients and families to keep them informed and engaged in decisions regarding their healthcare.
- Provides counseling, education, and resource information on wellness, prevention of illness, and health maintenance.
- Refers patients to specialty care services and educational training as appropriate for their conditions.
- Uses disease registries to identify gaps and out-of-range values in care delivery and utilizes support services and care managers to improve healthcare metrics.
- Participates in improving access to care for patients by supporting same-day appointments and extended hours of availability.
- Works with process improvement teams to enhance efficiencies of care delivery in the office setting.
- Maintains accurate and timely documentation of patient encounter records in accordance with coding guidelines, CMS, HIPAA, and other governmental regulations.
- Works with Practice Development and Improvement Managers to assist practices in achieving benchmarks toward Patient Centered Medical Home status and NCQA certification.
- Participates in clinical integration, process improvement initiatives, and coordination of care transitions between specialists and inpatient services.
- Provides timely follow-up after inpatient discharge to ensure effective transition to outpatient care.
