Job Description:
The Market Physician Executive (MPE) is a mission-driven physician leader responsible for overseeing Monogram Health’s in-home multi-specialty polychronic care model in an assigned market. Each market includes 5–10 practices led by advanced practice providers (APPs), registered nurses, licensed clinical social workers, and pharmacists. In this physician job, the MPE collaborates with Monogram Health’s multi-specialty platform to deliver in-home specialty care. The role combines leadership, population health management, and direct patient care responsibilities. This remote job also includes occasional in-home patient visits and domestic travel within the assigned market. This makes it an excellent environment for physicians looking for a meaningful MD job or leadership-focused DO job.
Qualifications:
- MD (Medical Doctor) or DO degree from an accredited medical school.
- Board Certified or Board Eligible in an ACGME approved specialty such as: Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology & Geriatrics
Additional qualifications include:
- Active and unrestricted state medical license.
- Willingness to obtain licenses in multiple states if required.
- Ability to obtain hospital privileges at designated facilities.
- Experience applying evidence-based clinical criteria.
- Experience in renal care and geriatrics.
- Experience working with Medicare Advantage and managed Medicaid populations.
- Knowledge of NCQA, HEDIS, quality improvement, medical utilization management, and risk adjustment.
Key Responsibilities:
Clinical Guidelines Execution
- Understand and implement Monogram Health’s evidence-based clinical pathways.
- Ensure adherence to established clinical guidelines and the Monogram Model of Care.
- Review and approve care plans developed by APPs, RNs, social workers, and pharmacists.
- Document patient clinical activities and interventions using Salesforce and Athena systems.
Clinical Operations and Quality Improvement
- Maintain accountability for reducing total cost of care and Medical Loss Ratio.
- Monitor clinical outcomes including interventions closure, utilization metrics, and therapy management.
- Collaborate with Market Managers to oversee patient engagement, scheduling, administrative oversight, and operational strategy.
- Present market performance and healthcare outcomes to senior leadership.
Patient Care and Treatment
- Provide both direct and indirect patient care, including diagnosis and treatment.
- Engage patients in treatment plans and evidence-based care pathways.
- Conduct peer-to-peer consultations with community physicians, facilities, and health plan partners.
- Order laboratory tests, referrals, and clinical interventions to improve patient outcomes and close care gaps.
