Qualifications:
- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) required
- Completed residency in Family Medicine or Internal Medicine
- Board Certification required
- Minimum two years of outpatient primary care experience preferred
- Active Illinois medical license, DEA, and Controlled Substance registration
- EHR competency; EPIC knowledge preferred
Physician Job Description:
This physician’s job involves providing the full scope of services under the provider’s field of training, including diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance. The role follows guidelines related to quality management, risk management, infection control, customer service, and safety programs established by Access Community Health Network.
Physician Job – Key Responsibilities:
General Clinical Responsibilities
- Obtains health history and performs physical examinations
- Diagnoses, treats, counsels, and coordinates care for patients
- Updates problem lists and longitudinal plans of care
- Completes documentation within 24 hours
- Prescribes medication according to Illinois statutes
- Meets Meaningful Use standards using certified EHR technology
- Manages EHR In-Basket and responds within 72 hours
Patient Support and Education
- Provides culturally competent care
- Recognizes families as part of the care team
- Encourages patient engagement and self-management
- Conducts patient education and follow-up instruction
- Coordinates with care teams and case management
PCMH Requirements
- Coordinates with health center staff
- Participates in team huddles and productivity standards
- Ensures timely, clinically appropriate patient care
- Supports effective care team performance
Provider Development
- Participates in staff training and orientation
- Ensures care team certification requirements
- Participates in proficiency testing
- May serve as a preceptor for medical students and residents
- Adheres to HRSA and FTCA regulations
