Medical Coder Job- Key Responsibilities
- Review and evaluate medical records, clinical notes, and discharge summaries for accuracy and completeness.
- Assign appropriate ICD-10 and CPT codes in compliance with CMS and HIPAA guidelines.
- Prepare clear and concise clinical summaries highlighting diagnoses, treatments, and patient outcomes.
- Identify gaps or inconsistencies in documentation and coordinate clarifications with clinical teams.
- Support claims processing, preauthorization, and medical necessity reviews with clinical insights.
- Collaborate with physicians, nurses, coders, and case managers to enhance documentation quality.
- Maintain strict confidentiality and ensure data integrity according to company and HIPAA policies.
- Participate in internal audits, process improvements, and coding accuracy training sessions.
Medical Coder Job- Qualifications & Experience
Education:

Bachelor’s degree in Nursing, Allied Health, Life Sciences, or related healthcare discipline.
Experience:
- 2–5 years of experience in medical coding, clinical documentation review, or utilization review.
- Strong knowledge of medical terminology, anatomy, physiology, and disease processes.
- Familiarity with EHR/EMR systems such as Epic, Cerner, or Meditech.
Preferred Certifications:

CPC (Certified Professional Coder) or CCS (Certified Coding Specialist) or equivalent credential.
